Course clinical guide → RSM Spine Mobility Assessment: Motion, Symptoms & Reassessment
Chronological age does not determine one massage routine. Start with the person’s health context, current clinical advice and the activity they want to manage. Avoid explanations that describe every older body as dehydrated fascia needing mechanical correction.

1. Clarify the Health Context Before a Manual Trial
Ask about recent injuries, known bone fragility, surgery, skin problems, relevant medicines and any current restrictions. Do not infer tissue tolerance from age alone or treat a short conversation as a complete medical screen.
Reviews of heterogeneous manipulation approaches offer limited evidence for older adults. They do not establish that a particular massage pressure is safe for someone with frailty, osteoporosis or a new unexplained symptom.
2. Find a Position the Client Can Enter and Leave
Compare comfortable sitting, side-lying or another supported position that fits the person’s needs. Support bony prominences and avoid demanding transfers merely to obtain your preferred access. Ask for contact feedback and change area, direction or duration before assuming more pressure is required.
Use a Meaningful Goal
Choose an ordinary activity the person wants to improve, such as a brief stand or putting on shoes. Keep the description and support consistent. A functional scale can inform monitoring, but an informal task check does not diagnose the source of the complaint.
3. Decide From Tolerance and the Same Activity
If a position or contact aggravates the familiar complaint, modify or omit it. New focal pain, marked function loss or a changing systemic complaint needs appropriate evaluation rather than a deeper adaptation.
Review the same agreed activity afterward and between visits. Better table comfort is useful in its own right; it does not establish improved balance, tissue hydration or prevention of falls.
